FREE regular teeth cleanings or periodontal maintenances (two per year)
FREE complete annual dental exam (up to two per year)
1 FREE additional exam per year(Limited Exam)
All x-rays needed to complete annual exam (BWx4 and Pano)
1 FREE Panoramic X-Ray every 5 years after ($169 value)
FREE teeth whitening (1 set of trays per member which includes 1 syringe of whitening gel, or 2 syringes of whitening gel)
A 20% savings on preventive, basic, and major dental procedures
A 15% savings implants placement, implant restorations and sleep appliances
This is a yearly cost, but monthly payments can be arranged as well for a $5.00 monthly service fee on enrollment
You work directly with our office!
There are no waiting periods, deductibles, maximums, or insurance hassles. QDP benefits begin immediately upon registration. All QDP Membership fees are due and payable at the time of registration and are non-refundable when services have been provided.
Plan duration is for one year from registration date. All patient portions for services received are due at time of services in order to receive QDP benefits, unless arrangements are made prior to appointment.
