FREE regular teeth cleanings or periodontal maintenances (two per year)
FREE complete annual dental exam (up to two per year)

All x-rays needed to complete annual exam (BWx4 and Pano)
1 FREE Panoramic X-Ray every 5 years after ($130 value)
FREE teeth whitening (1 set of trays per family member with 2 pack of gel at each renewal of plan)
A 20% savings on preventive, basic, and major dental procedures
A A 15% savings on implants placement, implant restorations and sleep appliances
This is a yearly cost, but monthly payments can be arranged as well for a $5.00 monthly service fee on enrollment

There are no waiting periods, deductibles, maximums, or insurance hassles. QDP benefits begin immediately upon registration. All QDP Membership fees are due and payable at the time of registration and are non-refundable when services have been provided.
Plan duration is for one year from registration date. All patient portions for services received are due at time of services in order to receive QDP benefits, unless arrangements are made prior to appointment. All members of a QDP family account will have the same anniversary date as the primary member. Interest-free automated payment plans up to 12 months duration may be available upon request with approved credit. Repayment duration is based on service totals and procedural type. A $5 processing fee will be added to each payment. A missed appointment fee of 25% of treatment total will be charged for all missed dental appointments. We reserve appointments to give you the best care and service possible, so please notify our office at least 24 hours in advance if you must change your reserved appointment.
